What's this test for?

This test is ideal if you're currently on TRT and need to monitor how your treatment is working. It helps check that your testosterone levels are in the right range, ensures your liver and kidneys are handling the treatment well, and monitors prostate health for any changes. Many men on TRT use this test every few months to make sure their hormone therapy is safe and effective.

How should I prepare for this test?

Fasting: 12-14 hours
No biotin: 72 hours
No ejaculation: 24 hours

You should fast for 12-14 hours before your test, which means no food or drinks except water during that time. Avoid ejaculation for 24 hours before the test for accurate PSA results, and if you're taking biotin supplements, stop them at least 72 hours before testing as they can interfere with results. If you use testosterone cream, make sure you haven't applied any to the inside of your elbow area for at least 24 hours, as this can cause falsely elevated results.

What's included in this test?

Sex Hormone Binding Globulin PSA, Total

This comprehensive follow-up panel checks your total and free testosterone to ensure your TRT dosage is working properly and measures SHBG which affects how much testosterone is available to your body. It monitors your complete blood count to watch for increased red blood cells (a common TRT side effect), checks liver and kidney function to ensure your body is handling the treatment safely, evaluates your lipid panel for cardiovascular health, and includes a PSA test to monitor prostate health. Together, these measurements provide a complete safety and effectiveness check for men on testosterone replacement therapy.

What happens after I get my results?
You'll receive detailed doctor notes in simple language explaining what your testosterone levels mean and whether your TRT dosage is working well for you. These notes include personalized recommendations about whether to adjust your treatment, lifestyle changes to enhance results, or if you should discuss dosage changes with your prescribing doctor. You'll also get the standard lab report and can track your hormone levels, liver function, and other markers over time using our trends feature to see how your TRT is progressing.
When will I get my results?
You can expect your results within 7 business days after your blood is drawn. If any confirmation testing is needed for unusual findings, it may take a bit longer, but this is rare. Most men get their complete results within a week.
Who should consider getting this test?
This test is specifically designed for men who are already on testosterone replacement therapy and need regular monitoring. It's recommended every 3-6 months while on TRT, or more frequently if you've recently changed your dosage or are experiencing side effects like mood changes, excessive energy, or concerns about prostate health. Your prescribing doctor will typically recommend how often you should test.
What symptoms indicate my TRT dosage might need adjusting?
Signs your TRT might need adjustment include feeling too energized or anxious (possibly too high), persistent fatigue or low libido (possibly too low), mood swings, difficulty sleeping, increased aggression, or water retention. Some men also experience breast tenderness, acne, or changes in urination frequency. If you're experiencing any of these symptoms, this test can help determine if your testosterone levels or other markers are out of range.
What lifestyle changes can help improve my TRT results?
To maximize your TRT benefits, focus on regular resistance training to build muscle and strength, prioritize 7-8 hours of quality sleep to support hormone balance, and maintain a diet rich in healthy fats, lean proteins, and vegetables. Managing stress through meditation or exercise helps prevent cortisol from interfering with testosterone. Limiting alcohol, staying hydrated, and maintaining a healthy weight also improve how your body responds to TRT.
Why is PSA testing important for men on TRT?
PSA (Prostate-Specific Antigen) monitoring is crucial for men on TRT because testosterone can potentially stimulate prostate growth. While TRT doesn't cause prostate cancer, it's important to monitor PSA levels to detect any prostate changes early. Regular PSA testing helps ensure your prostate remains healthy throughout your treatment and catches any concerns before they become serious.
What does high red blood cell count mean on TRT?
Elevated red blood cells (polycythemia) is one of the most common side effects of TRT. While some increase is normal and beneficial, too many red blood cells can make your blood thicker, potentially increasing the risk of blood clots or stroke. If your CBC shows elevated hematocrit or hemoglobin, your doctor may recommend donating blood, adjusting your TRT dosage, or temporarily pausing treatment.
How often should I retest while on TRT?
Most men on TRT should test every 3-6 months once their dosage is stable and levels are optimized. If you've recently started TRT or changed your dosage, your doctor may recommend testing more frequently, such as every 6-8 weeks, until you reach stable levels. Annual testing at minimum is essential even if you feel great, as some changes like increased red blood cells or altered liver function can occur without noticeable symptoms.

What this means

Your total testosterone level is above the optimal reference range, which may indicate your TRT dosage is higher than needed. While many men feel great at this level, sustained elevation can increase risks like elevated red blood cell count, mood changes, or cardiovascular strain over time.

* Regular blood test results (e.g., CBC) typically start arriving the next business day after sample collection. More complex tests, such as hormone panels, may take up to 10–15 business days due to their complexity.
